Employee Eligibility
Employees must be full-time to be eligible for benefits. New employees wishing to enroll in health, dental, or vision may do so within 30 days of hire. The benefits listed below, will start on the first of the month following the date of hire, unless otherwise specified. If you choose not to apply for benefits during the initial period of eligibility, you will have to wait until open enrollment in December of each year.
Employee Benefit Elections
Health insurance is 100% covered by fusionSpan for the employee (medical is covered 100% for the base plan, with a buyup charge for the lower deductible plan). Qualified additional relatives may be added to the plan at the employee’s expense (coverage cost is calculated based on the age of the participant, see HR for further details).

Automatic Enrollment - 100% Employer Covered Benefits
Long – Term Disability – Guardian
- 60% of monthly salary up to monthly max of $10,000
- Benefit begins on Day 91 of injury or illness
Life Insurance and Accidental Death & Dismemberment Insurance – Guardian
- Benefit amount is up to $100,000
- Benefit reduction of 35% at age 65 and 50% at age 70
Parental Leave
- After one year of full time employment
- Up to twelve weeks of unpaid leave
- Available PTO during the twelve week leave and short-term disability insurance
- After two years of full time employment
- Twelve weeks leave allowed
- 20 days paid leave in conjunction with available PTO and short-term disability insurance
PTO
All full-time employees are eligible to accrue a total of 17 days (136 hours) of PTO during their first 12 months of employment, PTO is accrued proportionately throughout the year. After each year of service, you will be eligible for an additional day of PTO, the total amount of PTO an employee can be eligible for in a year is capped at 22 days (176 hours).
